Sr/ /#d 16 October, 2008 To Advisory Group, National Heart Foundation on proposed Guideline on the Prevention of Infective Endocarditis Associated with Dental and Other Medical Interventions on behalf of the Paediatric Society of New Zealand Many thanks for circulating the new draft guidelines for review. Overall these look like a very sensible approach and we congratulate the committee on their work. The Paediatric Society of New Zealand endorses this important work by the National Heart Foundation. We would like however to make the committee aware of a few potential issues We would be keen to see that the comments relating to prescription of cover for the penicillin allergic (ie Table 3 For penicillin allergy or if a penicillin- or cephalosporin-group antibiotic taken more than once in the previous month) makes explicit that this group should include the group on long term penicillin prophylaxis including patients with rheumatic fever. In addition, for the Penicillin allergic/long tem penicillin group the guidelines recommend Clindamycin and Clarithromycin. There are problems in wide availability of these agents as below - Clindamycin 1. Only available as 150mg capsules and no elixir as noted (no good paediatric alternative for non capsule takers) 2. For > 3 caps, ie the 600mg dose, the script would need endorsement at the behest of a specialist which is somewhat unwieldy. We feel application should be made to Pharmac to allow up to 4 capsules to be funded without the necessity for endorsement if this is the NHF recommendation Clarithromycin 1. 250mg or 500mg doses would be available for capsule takers 2. The elixir according to our communications with Pharmac PHARMAC is only available via HEC application and then from a hospital pharmacy. If this is to be the alternative to amoxycillin for children then application needs to be made to PHARMACPharmac for this to be funded and easily available for this indication Obviously the relevant prescribing privileges need to be extended to the dentists in addition to medical practitioners. If these draft recommendations are accepted then we would be keen to see reissuing of the endocarditis cards carrying the revised advice. Thanks again for your work. Submissions

The Paediatric Society believes all children and youth should, by right, attain optimal physical, mental and social health and wellbeing.  By working as a coordinated national network of health professionals the Society dedicates its efforts and resources to making official submissions to both Government and Non-Government organisations on issues that will impact on the health and wellbeing of children and young people.
