However, in view of the specialist nature of the diet and the goal of minimising carbohydrate intake, it is essential that the supplements funded are appropriate for use with the diet. One of the supplements contains large amounts of carbohydrate (i.e. Paediatric Seravit) and therefore use of this supplement will significantly impact on the amount of total carbohydrate from food that the child is permitted, further reducing the palatability of the diet. For children receiving the ketogenic diet for epilepsy management only Ketovite tablets, Ketovite liquid and metabolic mineral mix should be used. None of these products contain carbohydrates. We recommend that Ketovite liquid should be fully funded for this group of patients. For some patients with metabolic disease receiving treatment with the diet, a higher carbohydrate intake may be permissible in some cases. Advice should be taken from the National Metabolic Service, led by Dr Callum Wilson, on this issue. The ketogenic diet is a third line treatment for children with refractory epilepsy. There is significant potential for adverse events during introduction of the diet. Patients stabilised on the diet require careful monitoring because of the metabolic and biochemical changes induced by the diet. The diet should be initiated by a multidisciplinary team which as a minimum should include a paediatric neurologist and a paediatric dietitian experienced in managing the ketogenic diet. We recommend that initial special authority applications for these supplements be restricted to named specialists, with renewals being permitted by any paediatrician. Submissions

The Paediatric Society believes all children and youth should, by right, attain optimal physical, mental and social health and wellbeing.  By working as a coordinated national network of health professionals the Society dedicates its efforts and resources to making official submissions to both Government and Non-Government organisations on issues that will impact on the health and wellbeing of children and young people.
