Survey: National Principles for Transition of Care for Rangatahi/Young People

PSNZ is undertaking a focused project to develop a concise set of National Principles for Transition of Care for Rangatahi/Young People.

A key priority of this mahi has been working in partnership with rangatahi/youth to co-design these principles, alongside consumers (whānau voice) and youth sector experts. The work aligns with international evidence and literature, while being specifically grounded in the needs, experiences, and aspirations of rangatahi/youth in Aotearoa New Zealand.

The principles outline the essential elements required to support effective transition programmes, resources, structures, and care pathways for rangatahi/youth moving from specialist services to primary care (e.g. General Practice). They are intentionally framed from a rangatahi/youth and whānau perspective to ensure transitions are developmentally appropriate, culturally responsive, and rangatahi/youth-centred.

As we move into the second phase of the project, we are seeking feedback from health and education providers across primary/community, secondary and tertiary settings. We want to understand whether the principles resonate and to identify what supports or enablers may be needed to embed them into practice.

Feedback will be collected via a short online survey open from 2 February to 28 February.
