Renée Streatfield, Paediatric Nurse Practitioner at True Colours Children's Health Trust in Hamilton, will share her experience developing a nurse-led clinic model designed to bring specialist paediatric care closer to home. The session will explore how integrated, collaborative approaches can improve access for rural communities and reduce unnecessary hospital presentations for tamariki with complex health needs.
